USFHP has New Emails, Direct Phone and Fax Numbers

USFHP has upgraded our internal systems to maintain security compliance with the Department of Defense.  As part this change, some of our contact info has changed. 

The resources below have been updated throughout our site. For those who may have saved our contact information, the changes are also summarized below to help you update your records.

What is staying the same?

Our website and 1-800 numbers will remain the same. That's great news, as most people will not see any change in how they reach USFHP:


For our providers and business partners, USFHP remains part of PacMed. Our legal name is not changing, so if you have a contract or agreement with us, these remain intact with no change needed. You can continue to call InstaMed Tech Support at 866-945-7990, and send invoices to the same @providence.org email as you have in the past. 

What is changing?

In short, if you have emailed with USFHP or called us using a direct phone/fax line (with a local 206 or 425 prefix), those contacts are all changing.

Our 2021 member handbook has been updated with our new contact information. Nothing else in the handbook has changed. If you would like the updated member handbook, you may call member services to request a paper copy, or download it here.

Individual contacts

If you have called or emailed with individual USFHP representatives, you can get their new direct email, phone or fax information by calling or emailing their old lines until March 24, 2021. You may receive an automated reply, so please be ready to take down the updated information provided. 

After March 24, you will need to contact Member Services above to request the new information.
