How to enroll in US Family Health Plan:
By phone
Call US Family Health Plan Enrollment Department at 800-585-5883, option 1
Online
Go to the TRICARE Beneficiary Web Enrollment (BWE) website select Manage health benefits and follow the prompts.
Note: TRICARE Prime beneficiaries may switch to USFHP at any time
Use ONLY the USFHP online provider directory to find a PCP in our network near you. Then call our Enrollment Department at 800-585-5883, option 1 to inform them about your choice of PCP

Download and mail your completed application form:
- TRICARE Prime application
- TRICARE Young Adult Program (TYA) paper application
- Allotment authorization form
