Chronic Disease Management Programs

US Family Health Plan is always striving to improve your care. Part of this is to treat chronic conditions aggressively. That’s why we use these two programs:

• Case management program: This is staffed by registered nurses (RNs). These nurses educate you about your illness and your treatment. They help you manage self-care and can recommend community resources. They also work with your doctor to coordinate your care with specialists and outside providers.
• Disease management programs: These are focused on diabetes and cholesterol conditions. There is also a pharmacy safety review program designed to help prevent negative drug interactions.

These special programs ensure that members get personalized information, assistance and advice. There is no extra cost involved for members.
